Use the following form, but remove the * symbols:

To take a screenshot of your lion, I suggest using lightshot or screenshu. You can also take a screenshot via your keyboard (depending on your system) and copy-paste the image to paint or any other graphic program. Then crop it the way you like, save on your computer, and host on image hosting sites like Imgur. Once you do this, grab "Direct image link" and paste it into the appropriate place in the form.
